At Cortex Neuro Rehab in Perth, we are dedicated to advancing the field of neurological physiotherapy. Recently, we conducted an enlightening study involving 15 stroke survivors and 10 carers, all of whom participated in or supported a modified constraint-induced movement therapy (mCIMT) program at home. Our aim was to map their experiences and perceptions to a behavioral framework, providing deeper insights into the effectiveness and acceptability of mCIMT in neurological rehabilitation.
Key Findings from Our Study:
Positive Perceptions of mCIMT: Stroke survivors were enthusiastic about mCIMT, noting significant improvements in upper limb function compared to other neuro rehabilitation interventions.
Structured Rehabilitation: Participants found that mCIMT provided much-needed structure to their daily routines as they adjusted to life post-stroke.
Achievable and Motivating: Despite its challenges, mCIMT was manageable, and the observed improvements in arm use motivated continued effort and engagement.
Demand for Extended Programs: All participants expressed a desire for longer mCIMT programs, challenging the notion that such therapy is too intense for stroke survivors.
Carer Observations: Carers noticed behavioral changes, with stroke survivors increasingly using their affected arm in daily tasks without prompting.
Carer Support: Contrary to concerns, mCIMT did not add to carer stress. Instead, it provided clear guidance and hope for future progress.
Implications for Neurological Rehabilitation:
Home-Based mCIMT is Feasible and Beneficial: Our findings indicate that mCIMT is not only acceptable but also advantageous in a home-based setting. It should be made available to all eligible stroke survivors.
Challenging Assumptions: Therapists should reconsider assumptions that mCIMT is too demanding or burdensome for carers. Instead, offering choice and flexibility to stroke survivors and their carers can lead to better outcomes.
Empowering Choices: It is crucial to empower stroke survivors and their carers by offering them the choice to participate in mCIMT, rather than making decisions on their behalf.
